You can ask to see or obtain a copy of your medical record. We will provide it electronically or by mail within 30 days of your request. Reasonable fees may apply.Request a correction

If you believe your privacy rights have been violated, you can contact us or file a complaint with the U.S. Department of Health and Human Services (HHS). You will not be penalized or retaliated against for filing a complaint.

You may tell us your preferences regarding how we share information in certain situations, such as:

Contacting you for appointment reminders or follow-up communications
If you are unable to communicate your preference, we may share information if it is believed to be in your best interest or required by law.
We will never use or share your information for:
Marketing purposes without your explicit written consent
Sale of your health information
3. Our Uses and Disclosures
We typically use or share your information in the following ways:
For Treatment
We use your information to provide, coordinate, or manage your medical care.
Example: A provider reviews your symptoms and prescribes treatment.
For Healthcare Operations
We may use information for administrative, training, and quality improvement purposes.
Example: Reviewing patient satisfaction or improving our services.
When Required by Law
We may share your information when required to do so by federal, state, or local law.
4. Cash-Pay Policy (No Insurance Billing)
Premier HealthCO is a cash-pay practice.
We do not bill insurance and do not submit claims to any insurance carrier, Medicare, or Medicaid.
You are responsible for payment in full at the time of service.
You may request an itemized invoice (superbill) for potential reimbursement from your insurance provider or HSA/FSA administrator. However, Premier HealthCO does not guarantee coverage, repayment, or reimbursement.
Because we do not contract with insurance companies, we will not disclose your health information to any insurance provider unless required by law or authorized by you in writing.
5. Additional Situations Where We May Use or Disclose Information
We may share your information when necessary for:
Public health reporting (for example, contagious disease notification)
Health oversight activities such as audits or investigations
Law enforcement or legal proceedings (subpoenas, court orders)
Coroners, medical examiners, and organ donation agencies (as required by law)
Research (only with appropriate authorization or oversight)
Preventing or reducing serious threats to health or safety
Workers’ compensation and occupational health reporting (if required)
6. Our Responsibilities
We are required by law to protect the privacy and security of your health information.
We must notify you promptly if a breach occurs that may compromise your privacy or security.
We must follow the duties and privacy practices described in this Notice.
We will not use or share your information other than as described here, unless you authorize us in writing. You may revoke authorization at any time.
7. Telehealth and Electronic Communication
If you receive care through telehealth, your information may be transmitted electronically using secure, encrypted platforms.
Premier HealthCO uses HIPAA-compliant technology that meets federal standards (45 CFR §164.312).
While we employ strict safeguards, you acknowledge that no electronic system is completely risk-free, and you consent to electronic communication for treatment, scheduling, and follow-up purposes.
8. Marketing, Fundraising, and Sale of Information
Premier HealthCO does not sell, rent, or trade your personal health information.
We may contact you to provide appointment reminders, updates about available services, or optional health information. You may opt out at any time.
9. Changes to This Notice
We may revise this Notice periodically. Updates will apply to all information we maintain.
