Weight Loss & Peptide Therapy

At Premier HealthCO, we offer select services—including medical weight loss and peptide therapy—through asynchronous care. This means your treatment plan may be prescribed without a live phone or video appointment, based on your completed intake form, medical history, and any required labs.

How Asynchronous Care Works

After you complete our secure medical intake form, a licensed provider will review your health information and determine whether the requested treatment is appropriate and safe. If approved, your prescription will be sent to a licensed pharmacy and shipped directly to your door.

You’ll receive personalized instructions and ongoing access to our care team for questions or follow-up.

Please note: Submitting an intake form does not guarantee treatment. All prescriptions are issued only when deemed medically appropriate after a provider review.

Who May Not Be Eligible

You may not qualify for certain therapies if you:

• Are currently pregnant or breastfeeding

• Have active or recent cancer

• Have uncontrolled medical conditions such as diabetes, kidney disease, liver disease, or heart disease

• Are taking medications that may interact with weight loss or peptide therapies

• Are under the age of 18

• Providing false information on the intake form will automatically result in denial

Your provider may request additional labs or documentation before approving treatment.

Chart Review Fee & Policy

All asynchronous submissions are subject to a $30 provider review fee. This fee covers time spent reviewing your chart, assessing treatment eligibility, and completing medical documentation.

This fee is non-refundable, even if treatment is not approved based on your health history or provider discretion.

North Dakota (ND)
Oregon (OR)
Utah (UT)
Washington (WA)
Wyoming (WY)

By submitting your intake form, you confirm that you are physically located in one of the states listed above at the time of submission. Patients outside these states are not eligible for asynchronous care and will forfeit the review fee.

Consent to Electronic Communication & Telehealth

By using our asynchronous care services, you consent to receive care electronically, including:

• Medical decision-making based on online forms

• Treatment plans, prescription delivery, and follow-up messaging via email or SMS

• Ongoing digital communication with our licensed providers and care team

You acknowledge that asynchronous care may have limitations compared to in-person or live virtual visits.

All services are provided by licensed healthcare professionals using their independent clinical judgment. Treatments—including compounded peptides and weight loss medications—may be prescribed for off-label use and are not FDA-approved to treat, cure, or prevent any specific disease unless explicitly stated.

Premier HealthCO does not accept patients enrolled in Medicare or Medicaid.
