FDA Notice & Off-Label Use
Some medications, peptides, or therapies offered by Premier HealthCO may be prescribed for off-label use. These treatments have not been evaluated or approved by the U.S. Food and Drug Administration (FDA) for all indications. They are not intended to diagnose, treat, cure, or prevent any disease. Results may vary. No guarantees are made regarding individual outcomes.

Medical Oversight & Provider-Patient Relationship
All treatments require an evaluation by a licensed healthcare provider. A provider-patient relationship is not established until a formal consultation and signed consent occur. Use of this website or submission of any information does not constitute a provider-patient relationship.

Medication Sourcing
Medications are sourced from either:

  • Licensed U.S. commercial pharmacies

  • FDA-registered 503A or 503B compounding pharmacies

Compounded medications are made specifically for individual patients based on a provider’s prescription and may not be FDA-approved.

HIPAA & Privacy Compliance
Premier HealthCO complies with the Health Insurance Portability and Accountability Act (HIPAA). Any health information submitted is encrypted, stored securely, and only accessible to authorized clinical staff. We do not sell or share personal data. For more information, see our Privacy Policy.

Pricing & Payment Transparency
Treatment pricing may include:

  • Medications

  • Provider consultations and follow-up

  • Supplies and equipment

  • Shipping (if applicable)

  • Medical management

Premier HealthCO does not accept insurance. Patients with Medicare or Medicaid are not eligible for treatment. FSA/HSA cards are accepted; however, patients are responsible for verifying eligibility. Receipts and superbills can be provided upon request.

Billing, Refund & Appointment Policy

  • No refunds are issued once a prescription is submitted to the pharmacy. Medications cannot be returned.

  • A $75 no-show fee applies to missed appointments without 24-hour notice.

  • A $45 fee applies to asynchronous consultation reviews and is non-refundable, even if the patient is not eligible for treatment.

  • By submitting payment, you agree to our billing policies and authorize Premier HealthCO to charge the provided card for services rendered.

We are currently authorized to treat patients located in the following states:
AZ,CO, FL, HI, IA, KS,NE, NV,ND,OR,UT, WA,WY

ONLY TESTOSTERONE ELIGIBLE STATE: In-person exams at the Kansas location- no telehealth for Testosterone HealthCO patients

Patients are responsible for confirming eligibility before submitting an intake or payment. If a patient submits from a non-covered state, the review fee is non-refundable.

Any lab testing arranged through this link is conducted independently by the third-party laboratory provider.

Results obtained through BuyLabWork are for informational and educational purposes only and do not establish a provider–patient relationship with Premier HealthCO.
Our clinical team will only review or interpret lab results after a formal consultation, consent, and onboarding process have been completed.

If you choose to order labs through this service, please download and retain a copy of your results for your own records and bring them to your initial consultation for review.
